Your initial appointment will consist of a consultation explaining your diagnosis and treatment options. Occasionally, treatment can be done the same day as the consultation. However, complex cases may require a second visit.

Please assist us by providing the following information at your consultation:

• Your referral slip and any X-rays if applicable.
• Driver’s License and Insurance Card.
• A list of medications you are currently taking.
• Completed insurance forms if applicable.

IMPORTANT:

A parent or guardian must accompany all patients under 18.

Please notify the office if you have any medical conditions or are taking medications that may affect your treatment, including heart conditions, diabetes, blood pressure concerns, or anticoagulant therapy.
